Regenexx is covered as an in-network benefit within the Xplor health plans. In-network benefits for specialist services within your plan and in-network copay and deductible apply for the Regenexx evaluation. For the OAP and PPO plans, a copay of $250 applies to each Regenexx procedure. Amounts in excess of the $250 copay per treatment will be covered at 100% under the OAP and PPO plans. For the HDHPQ plans, the in-network deductible will apply, amounts in excess of the applicable deductible are covered at 100%. Non-Regenexx services may fall under a different benefit level, and may or may not be treated as in-network.
